IS Account Support Request

Service Description

This form should be used to submit requests to the Information Services Technology Access and Support staff. Before submitting this form, please search the Service Catalog for an entry that matches your request. If you are unable to find one, then submit your request here.

Some examples of items that should be submitted through this form:

  • Cognos/IDR Student Data and Finance and HR Data forms
  • Resource25 and 25Live account requests
  • Legacy Data Warehouse access
  • Exchange distribution list aliases
  • Talisma aliases
  • Department mapping requests
  • Questions regarding eligibility for services
  • Username change questions
  • Requests to add authorized signers
  • Requests for remote Banner access
  • General questions about accounts and access
