IS Account Support Request

Service Description

This form should be used to submit requests to the Information Services Technology Access and Support staff. 

Before submitting this form, please search the Service Catalog for an entry that matches your request. If you are unable to find one, then submit your request here.

Some examples of requests that should be submitted through this form:

  • Cognos/IDR Student Data and Finance and HR Data forms
  • Resource25 and 25Live account requests
  • Legacy Data Warehouse account requests
  • Requests to close business application accounts for terminated employees (such as Banner, Cognos/IDR, Singularity Imaging, etc)
  • Exchange distribution list aliases
  • Talisma aliases
  • Department ADOU mapping requests
  • Questions regarding eligibility for services
  • Username change questions
  • Requests to add authorized signers
  • Requests for remote Banner access
  • Requests for Banner TEST access
  • General questions about accounts and access
