How to Request Support

To request support select the request type.

Employees or supervisors can request login access for Banner, Appworx, and DuckDocs

Submit requests to the Information Services Technology Access and Support staff.

Login and data access can be requested by an employee's supervisor, dean, department head, or designee for IDR/Cognos.

Request help with Duck ID accounts.

Request help with trouble claiming a Duck ID.

Recover your personal Duck ID password or request a temporary Duo bypass code.

Request a new role or department account, or update an existing account.

Request consultation related to applications that are being considered for purchase that may require UO single sign-on (SSO).

Faculty and staff can use this service to request a Sponsored Duck ID.

Request support for two-step login (Duo).

Register a Duo token or request registration of alternate tokens.

Request an update to your campus phone number or address directory information.
