Requirements
- Information about user
- Full name
- UO ID (95 number)
- Prox ID of user's UO ID card (found on back of UO ID card, in the top right)
- User's job/role
- All external contractors, vendors, and clients must display identification when on University of Oregon property.
- Housing managers overseeing work and projects involving external contractors and vendors must request contractor badges through ID Card Services. The Card Office will notify you when the badges are ready. When badges are ready and picked up, a ticket for access request should be made through this service offering.

You can expect a response to your request within 1 business day.

If you are experiencing a physical problem with a door (lock is malfunctioning, door will not close, card reader malfunctioning) please call the Housing Duty Admin at 541-913-0043.
