All Campus Faculty and Staff may use this service to request support of Forms.uoregon.edu functions included but not limited to:
- Report a problem
- Request new form/schedule consultation
- Request Modifications to Existing Forms
- Request changes to data settings
How to Create a Ticket
To request support click Create a Ticket (top right of the page) and provide:
- Module (UO Forms)
- Are you requesting a fix, update, new form, or new report
- Provide details that explain the request or issue
- Request a new form:
- Name of the form
- Retention series/risk level
- Optional: attach example form for evaluation
- Requested Completion Date
- Request a new report:
- Name of the Document Type(s)
Availability and Access
You can expect a response to your request within 24hrs.
