UO Forms Support Request

All Campus Faculty and Staff may use this service to request support of Forms.uoregon.edu functions included but not limited to:

  • Report a problem
  • Request new form/schedule consultation
  • Request Modifications to Existing Forms
  • Request changes to data settings

How to Create a Ticket

To request support click Create a Ticket (top right of the page) and provide:

  • Module (UO Forms)
  • Are you requesting a fix, update, new form, or new report
  • Provide details that explain the request or issue
  • Request a new form:
    • Name of the form
    • Retention series/risk level
    • Optional: attach example form for evaluation
    • Requested Completion Date
  • Request a new report:
    • Name of the Document Type(s)

Availability and Access

You can expect a response to your request within 24hrs.
