UHS personnel use this service to request access to UHS Clinical accounts/applications, including but not limited to:

• EHR
• Pharmacy
• Laboratory
• Other (please list)

How to Request Access

To request support, click Request Access(top right of the page) and provide the requested information.

1. Title (short description of your request)
2. Application name
3. Supervisor/Manager name (the person who needs to approve the request)
4. Due Date (when do you need access)
5. Description (any details of your request)

Availability and Access

You can expect a response to your request within six business hours.
