UHS personnel use this service to request access to UHS Clinical accounts/applications, including but not limited to:
- EHR
- Pharmacy
- Laboratory
- Other (please list)
How to Request Access
To request support, click Request Access(top right of the page) and provide the requested information.
- Title (short description of your request)
- Application name
- Supervisor/Manager name (the person who needs to approve the request)
- Due Date (when do you need access)
- Description (any details of your request)
Note: You must identify the person who can approve your request.
Availability and Access
You can expect a response to your request within six business hours.
