Use this service to report Suspected Identity Theft (Red Flag). The data will be compiled and reported as outlined in the UO Identify Theft Prevention Program (ITPP) that is required by the Federal Trade Commission (FTC) Red Flags Rule.

Provide all relevant information that occurred in this incident.

How to Create a Ticket

To report click Create a Ticket (top right of the page) and provide the following information.

  1. Title
  2. Date and time of the incident (mandatory)
  3. UO ID and name of individual whose identifying information was used fraudulently
  4. Specific identifying information or documents that were used fraudulently. (mandatory)
  5. Description of the incident
