Upload File option greyed out on Microsoft Forms

Issue

The Upload File question option is greyed out in Microsoft Forms. The form is correctly set to be only accessible to UO community members. Brand new forms can be created that have this functionality available.

Hello, I am trying to add the upload image feature into a form while keeping the form with it’s current setting to be open to UO folks only so that they sign in, but I cannot insert the upload image feature to our Microsoft form. It’s grayed out.

Resolution

The owner (almost always the creator) of the form must add any Upload File questions since any files uploaded to the form will be stored in either the owner's OneDrive or SharePoint.

Unfortunately, collaborators on MS Forms do not have the ability to add Upload File questions. If a form is shared with you, you are a collaborator on the form. If you are unsure, you can follow these steps to check ownership:

  1. While on the main screen from which you can edit the form, click the triple dot icon in the upper right corner of the browser window.
  2. Select Collaborate or Duplicate from the drop-down menu.
  3. Next to the profile picture icons under the Add collaborators section, click the triple dot icon labelled More.
  4. This should list all users who have access to the form and indicate who has ownership.
