UO Staff, Faculty, and Students use this service to report an IT Security Concern. Report actual or suspected IT security incidents as soon as possible so that work can begin to investigate and resolve them.
Attention: If the incident poses any immediate danger, call 911 to contact law enforcement authorities immediately.
Note: If, for any reason, you are unable to fill out this form, concerns may also be reported by emailing infosec@uoregon.edu, or by calling 541-346-5837.
Some examples of potential information security concerns you may report are not limited to, but may include the below:
- Suspected or actual security breaches of information – whether in printed, verbal, or electronic form – or of information systems used in the pursuit of the university's mission.
- Abnormal systematic unsuccessful attempts to compromise information – whether in printed, verbal, or electronic form – or information systems used in the pursuit of the university's mission.
- Suspected or actual weaknesses in the safeguards protecting information – whether in printed, verbal, or electronic form – or information systems used in the pursuit of the university's mission.
How to Report Concern
To request support click Report Concern (top right of the page) and provide the requested information.
- What happened (A clear explanation of the security concern, including what was observed or experienced)
- When did the concern occur, or when did you notice it? Please Include details in the description of the concern
- Where is the concern occurring?
- On a Computer System or Server
- An application or cloud based application
- On the network
- Unknown
- What was the affected System(s) or application(s)
- Potential Impact (if known)
- Please attach any evidence you may have about the concern you are reporting. Any supporting information like
- screenshots
- error messages
- logs
- relevant data samples will help further determine the impact.
- Mitigating actions taken (Have you taken any steps to address this concern? If so, please provide details)
- Are there steps that can be taken to recreate the issue?
Please provide as much information as you are able to; all of the information will help the Information Security Office acurately assess the situation and take action to reduce risk.
