How to Request Phone

To request support click Request Phone (top right of the page) and provide the requested information.

  1. Phone type (elevator, call station, landing phone)
  2. One time cost index
  3. Monthly charge index
  4. If elevator phone
    1. location(s) of elevator panel(s)
    2. jack number of panels (corresponding to the panel locations)
  5. If call station
    1. number of new exterior call stations
    2. jack number(s) of external locations
  6. if elevator landing
    1. location of interior call station(s)
    2. jack number(s) of internal locations
  7. Description/additional information
