- Are/were you an Oregon resident for tuition purposes?
- Did you apply to the UO as a degree-seeking student?
- If yes, what was your intended major when you applied
- Which high school did you attend?
- Have you applied/did you apply to graduate with an undergraduate degree?
- Can you name any prior colleges you attended before UO?
- Last four digits of the Social Security number
