How to Submit a Ticket

    2. How many credit hours are you currently registered for?
    3. Are/were you an Oregon resident for tuition purposes?
    4. Did you apply to the UO as a degree-seeking student?
      1. If yes, what was your intended major when you applied
    5. Which high school did you attend?
    6. Have you applied/did you apply to graduate with an undergraduate degree?
    7. Can you name any prior colleges you attended before UO?
  10. Last four digits of the Social Security number
