Use this service to report an outage on one of the COVID applications.
How To Report an Outage
If the Mazik application, Mazik public registration or resulting pages, L7 LIMS, IS SFTP or other COVID MAP associated services are unavailable outside of communicated downtimes, click on the Report Outage button on right side of the page to report a service outage and provide the following information:
- Description of the Outage
- When the service was last working for you
- Any screenshots or other documentation
Availability and Access
UO Affiliation (faculty, staff, and some students) whom are working on or as part of a COVID-19 Monitoring and Assessment Program (MAP) project.
Service outages reported on this page are monitored during business hours only (8am to 5pm), Monday through Friday. See below for more information about reporting service outages after business hours.
