How to Create a Ticket

To request support click Create a Ticket (top right of the page) and state whether this is an:

  • Request access to the FMLA application or the ADRF mailbox
  • Issue (something is broken)
  • Request a change to the application or form configuration

Availability and Access

Available to approved Human Resources staff.

You can expect that this service request to be completed in 1-5 business days, depending on the scope of the request.
